To participate in this program, an assessment by Home and Community Care Support Services (HCCSS) is necessary. This assessment determines the patient's needs and their eligibility for the program, including developing a personalized service plan.

The Family-Managed Home Care program provides eligible patients—or their substitute decision-makers—with funding to hire care providers or purchase home care services directly. This approach offers greater flexibility and choice, allowing families to select their own care providers, determine how and when care is delivered, and manage all related administrative responsibilities, including provider coordination, funding oversight, and reporting.
To participate, patients must first undergo an assessment to determine their care needs, meet both general and specific eligibility criteria for traditional home care, and have a service plan developed by Ontario Health at Home.
This program is available to patients in the following categories:
Children with complex medical needs
Adults with acquired brain injuries
Patients facing extraordinary circumstances
Substitute decision-makers must also be assessed to confirm they are capable of managing the responsibilities of the program, including coordinating care, overseeing employment of providers, and handling financial and reporting requirements.
